Preparing for Your Visit

Pre-Admission COVID-19 Testing   |  Guest Policy

We understand that your time is valuable, and want your visit to be as efficient as possible. Please complete and print the registration form and pregnancy questionnaire below prior to your appointment and review the other documents.

The San Diego Perinatal Center participates with many quality health plans. For information on accepted health plans at our center, please call 858-966-6710.

Our main location at 7910 Frost Street is adjacent to Sharp Mary Birch Hospital for Women and Rady Children’s Hospital-San Diego.

Day of Your Visit

On the day of your appointment, please be sure to bring the following items with you:

  1. Current insurance card
  2. Valid government-issued photo ID
  3. Referral form from your doctor’s office
  4. Your completed registration form  (English|Spanish)

Ultrasound examinations and procedures may require a full bladder.  We ask that you drink two glasses of water before your visit, and try not to void until you are seen.

Given the decrease in COVID cases in San Diego County, we are able to allow 1 adult guest to accompany patients to their appointments. No Children will be permitted to appointments at this time.
